Microstructural Abnormalities in Gray Matter of Patients with Postherpetic Neuralgia: A Diffusional Kurtosis Imaging Study.
نویسندگان
چکیده
BACKGROUND Changes in functional activity and connectivity have been shown in patients experiencing postherpetic neuralgia (PHN) pain. However, PHN-induced structural changes, particularly in the gray matter of which volume and density was widely reported to be altered by other chronic pain, have not been well characterized. OBJECTIVE In this study, we aimed to detect the difference in the microstructure of gray matter of PHN patients as compared to the healthy controls, and to analyze the correlation between microstructural alterations and clinical features of PHN patients. STUDY DESIGN Observational study. SETTING University hospital. METHODS Diffusional kurtosis imaging (DKI) was performed in 19 patients with PHN and in 19 age- and gender-matched healthy controls. Maps of axial kurtosis (K//), mean kurtosis (MK), radial kurtosis (K) in gray matter were calculated and compared between the 2 groups. Correlations between kurtosis metrics in the regions where between-group difference was detected and pain intensity as well as lesion duration were tested by Pearson's correlation. RESULTS Compared with healthy controls, PHN patients exhibited significantly decreased DKI parameters in the bilateral insula and superior temporal gyrus, left middle frontal gyrus and occipital lobe, right cerebellum anterior lobe, right thalamus, caudate and parahippocampal gyrus. K// in the bilateral insula and MK in the right insula were negatively correlated with visual analogue scale (VAS) scores of PHN patients, whereas no correlation was found between DKI parameters and lesion duration of PHN pain. LIMITATION Relatively small sample size. We still cannot determine the causal and effect relationship between the microstructural abnormalities in the gray matter and PHN. CONCLUSIONS DKI can specifically reflect pathophysiological microstructural alterations in the cerebral gray matters of PHN patients. This feature enables magnetic resonance imaging (MRI) to be a potentially valuable technique for objectively estimating the severity of PHN pain, which would provide an opportunity for elucidating the central mechanisms underlying PHN as well. KEY WORDS Postherpetic neuralgia, diffusional kurtosis imaging, insula cortex, gray matter, voxel-based analysis.

Altered microstructure in temporal lobe epilepsy: a diffusional kurtosis imaging study.
BACKGROUND AND PURPOSE Temporal lobe epilepsy is associated with regional abnormalities in tissue microstructure, as demonstrated by DTI. However, the full extent of these abnormalities has not yet been defined because DTI conveys only a fraction of the information potentially accessible with diffusion MR imaging. In this study, we assessed the added value of diffusional kurtosis imaging, an ex...
متن کاملMicrostructural Abnormalities of Basal Ganglia and Thalamus in Bipolar and Unipolar Disorders: A Diffusion Kurtosis and Perfusion Imaging Study
OBJECTIVE Bipolar disorder (BD) is often misdiagnosed as unipolar depression (UD), leading to mistreatment and poor clinical outcomes. However, little is known about the similarities and differences in subcorticalgray matter regions between BD and UD. METHODS Thirty-five BD patients, 30 UD patients and 40 healthy controls underwent diffusional kurtosis imaging (DKI) and three dimensional arte...
متن کاملMicrostructural Abnormalities Were Found in Brain Gray Matter from Patients with Chronic Myofascial Pain
Myofascial pain, presented as myofascial trigger points (MTrPs)-related pain, is a common, chronic disease involving skeletal muscle, but its underlying mechanisms have been poorly understood. Previous studies have revealed that chronic pain can induce microstructural abnormalities in the cerebral gray matter. However, it remains unclear whether the brain gray matters of patients with chronic M...
متن کاملA better characterization of spinal cord damage in multiple sclerosis: a diffusional kurtosis imaging study.
BACKGROUND AND PURPOSE The spinal cord is a site of predilection for MS lesions. While diffusion tensor imaging is useful for the study of anisotropic systems such as WM tracts, it is of more limited utility in tissues with more isotropic microstructures (on the length scales studied with diffusion MR imaging) such as gray matter. In contrast, diffusional kurtosis imaging, which measures both G...
متن کاملMean Diffusional Kurtosis in Patients with Glioma: Initial Results with a Fast Imaging Method in a Clinical Setting.
BACKGROUND AND PURPOSE Diffusional kurtosis imaging is an MR imaging technique that provides microstructural information in biologic systems. Its application in clinical studies, however, is hampered by long acquisition and postprocessing times. We evaluated a new and fast (2 minutes 46 seconds) diffusional kurtosis imaging method with regard to glioma grading, compared it with conventional dif...
